Make sure you have read and understood which forms are required and that the correct procedures are followed filling them out. Please make sure you send the forms in with your booking form or  prior to the jump date.

Tandem Medical Forms

There are TWO DIFFERENT TANDEM MEDICAL forms.

EVERYONE doing a tandem skydive has to fill out ONE of these medical forms. We must receive this prior to the jump date if you haven’t sent it in with your booking form.

Please read carefully the Student Tandem Skydiver Medical Information and Declaration form (form 115A first red click link) to see which one you need to fill out.

If you have not suffered in the past or do not currently suffer from any of the conditions listed on the Student Tandem Skydiver Medical Information and Declaration form (115A) you may fill in this form yourself without going to the doctor.
If you have had in the past or do currently suffer from any of the conditions listed on the Student Tandem Skydiver Medical Information and Declaration Form (115A) then you must get your doctor to sign & stamp the OTHER form called the –

STUDENT TANDEM SKYDIVER REQUEST FOR MEDICAL ADVICE (FORM 115B)

A LETTER FROM YOUR DOCTOR IS NOT ACCEPTABLE. THE DOCTOR MUST ONLY SIGN & STAMP THE STUDENT TANDEM SKYDIVER REQUEST FOR MEDICAL ADVICE (FORM 115B) IF REQUIRED. THEY MUST NOT WRITE ANY EXTRA INFORMATION ON THIS FORM AS IT WILL INVALIDATE IT.

If you suffer from Asthma there is an extra form that you will need to take along with the STUDENT TANDEM SKYDIVER REQUEST FOR MEDICAL ADVICE (FORM 115B) to your GP to read.

PLEASE NOTE – It is our policy not to allow anyone who uses or relies on a wheelchair to undertake a tandem skydive. There is no upper age limit for the tandem skydives, however if you are aged 60 or over we will require you to attend the centre for a pre-assessment before accepting a booking.

If you rely upon walking aids for your mobility then we will require you to attend the centre for a pre-assessment before accepting a booking.

Pre assessments are normally done on a Monday or Friday.

Application for Student Provisional Membership & Agreement

The British Skydiving Application for Student Provisional Membership & Agreement needs to be filled in by everyone doing a jump. If the person jumping is 16 or 17 years old then they must also get a parent/guardian to fill out and sign the relevant sections too. This form must be sent in prior to the jump date if not alreay sent in with the booking form. The centre will have spare forms at check in but if the person jumping is 16/17 years old they must have a parent/guardian to fill out and sign the relevant sections for them.

Solo Student Medical Forms

EVERYONE has to fill out a Solo Student Medical Form. There are two different Medical Forms.

Form 115C Solo Student Parachutist Self- Declaration of Fitness to Parachute.

Form 115D Solo Student Parachutist Request For Medical Advice.

You need to read Form 115C first .

If nothing listed on Form 115C applies to you then you can fill in Form 115C yourself. If you are 16/17 years old a parent/guardian will need to witness this form.

If something listed on Form 115C applies to you then you must take the other Medical Form 115D to your Doctor to have it signed and stamped by them.

A letter from your doctor is NOT acceptable.
